I am aware that in sports activity, the possibility of serious and catastrophic injury is present. I voluntarily assume all risks of loss or injury that my child may sustain from participation in the Girls Soccer Worldwide™ Leadership Workshops, including but not limited to sports activities.

I release, discharge, and waive all claims towards Girls Soccer Worldwide and/or its owners, officers, directors, employees, agents, and personnel from any and all liability for any damage or injury which results from my child’s participation including liabilities that arise from the negligent acts and/or omission of Girls Soccer Worldwide.

I give Girls Soccer Worldwide™ the right and permission to copyright, use and publish photographic/video images of my child which include, but are not limited to, any printed materials or any social media format. I relinquish any rights to examine or approve prior to use.

I understand Girls Soccer Worldwide™ has preventative measures in place to reduce the spread of COVID-19, however, Girls Soccer Worldwide™ cannot guarantee the participant will not become infected with COVID-19.

I understand Girls Soccer Worldwide™ may revise its procedures at any time based on updated recommended guidance and recommendations issued by public health agencies and further agree to comply with revised procedures.

1) I agree I will not allow my daughter to attend any Girls Soccer Worldwide™ activities if she has TESTED POSITIVE for COVID-19 within 5 days of any scheduled workshops. I agree she will not return until a negative test result is received.

2) I agree my daughter will not attend workshops if she has any of the following SYMPTOMS: sore throat, cough, chills, body aches for unknown reasons, shortness of breath, loss of smell, loss of taste, fever at or greater than 100 degrees Fahrenheit.

I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child and I may be exposed to or infected by COVID-19 by attending Girls Soccer Worldwide™ programs and that such exposure or infection may result in personal injury, illness, permanent disability, and death.

I grant permission to Girls Soccer Worldwide™ to notify and authorize medical personnel to treat all injuries occurring on the premises. I acknowledge that I am responsible for paying for any medical treatment that my child may receive, as a result, of participation in Girls Soccer Worldwide™ programs.
