Join us for our upcoming soccer and empowerment workshops in Livermore. Each workshop consists of 30 minutes of soccer + 30 minute leadership workshops that focus on one social and emotional skill each week. Each activity will be hands-on and centered around empowering messages.
WHO: OPEN to girls currently in 4th-6th grade (open to girls in 3rd grade is older sibling is attending)
WHEN: 1/15, 1/22, 1/29, 2/5
TIME: 3:30pm-4:30pm
Arrive 10 minutes early to check in. Masks are required.
Wear sneakers, bring water and soccer ball. (If you don't have one of your own you are welcome to use one of ours.)
Please read each statement below in its entirety. To approve, check the box next to each statement.
I am aware that in sports activity, the possibility of serious and catastrophic injury is present. I voluntarily assume all risks of loss or injury that my child may sustain from participation in the Girls Soccer Worldwide™ Leadership Workshops, including but not limited to sports activities.
I grant permission to Girls Soccer Worldwide™ to notify and authorize medical personnel to treat all injuries occurring on the premises. I acknowledge that I am responsible for paying for any medical treatment that my child may receive, as a result, of participation in Girls Soccer Worldwide™ programs.
I release, discharge, and waive all claims towards Girls Soccer Worldwide and/or its owners, officers, directors, employees, agents, and personnel from any and all liability for any damage or injury which results from my child’s participation including liabilities that arise from the negligent acts and/or omission of Girls Soccer Worldwide.
I give Girls Soccer Worldwide™ the right and permission to copyright, use and publish photographic/video images of my child which include, but are not limited to, any printed materials or any social media format. I relinquish any rights to examine or approve prior to use.
I acknowledge that within 48 hours of my child attending any Girls Soccer Worldwide™ activity I am required to self-certify on their behalf for the below COVID related statements.
I, nor anyone in my household have had any of the following symptoms in the last 21 days: sore throat, cough, chills, body aches for unknown reasons, shortness of breath for unknown reasons, loss of smell, loss of taste, fever at or greater than 100 degrees Fahrenheit.
I, nor anyone in my household have tested positive for COVID-19 in the past 21 days. To the best of my knowledge, I nor anyone in my household, have not been in close proximity to any individuals who tested positive for COVID-19 in the past 21 days. I do not have any reason to believe myself or anyone in my household has been exposed to or acquired COVID-19.
I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child and I may be exposed to or infected by COVID-19 by attending Girls Soccer Worldwide™ programs and that such exposure or infection may result in personal injury, illness, permanent disability, and death.
